Liability Waiver & Release
In consideration of participating in the yoga classes at Fusion Hot Yoga; 22618 Bothell-Everett Highway, Bothell, WA 98021, I represent and agree as follows:
If this waiver is being completed on behalf of a minor, the parent or legal guardian is accepting these terms on the minor's behalf.
- That I am participating in yoga classes conducted in a heated room with elevated humidity. I understand this environment may be physically demanding and I am fully aware of the risks involved, including those related to heat, physical exertion, and personal injury.
- That it is my responsibility to consult a physician prior to and regarding my participation. I represent that I am physically fit to participate and have disclosed any known medical conditions in the form below.
- That Fusion Hot Yoga is in no way responsible for safekeeping of my personal belongings while I attend class.
- That classes at Fusion Hot Yoga may be physically strenuous and I voluntarily participate in them with full knowledge that there is a risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Fusion Hot Yoga or its members for any personal injury, property damage/loss, or wrongful death, whether caused by my negligence or otherwise.
