New Student Waiver

Liability Waiver & Release

In consideration of participating in the yoga classes at Fusion Hot Yoga; 22618 Bothell-Everett Highway, Bothell, WA 98021, I represent and agree as follows:

1. That I am participating in the yoga classes, during which I will receive information and instruction about yoga and health. I recognize that yoga requires physical exertion that may be strenuous and am aware of the hazards and risks involved.
2. That it is my responsibility to consult my physician prior to and regarding my participation in the yoga classes. I represent and warrant that I am physically fit and have no medical conditions that would prevent my full participation in the yoga classes.
3. That Fusion Hot Yoga is in no way responsible for safekeeping of my personal belongings while I attend class.
4. That classes at Fusion Hot Yoga may be physically strenuous and I voluntarily participate in them with full knowledge that there is a risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Fusion Hot Yoga or its members for any personal injury, property damage/loss, or wrongful death, whether caused by my negligence or otherwise.
